We presented a case with pseudoaneurysm of the ascending aorta, diagnosed two years after coronary artery bypass grafting (CABG), and explained our surgical approach. A 47-year-old male who had CABG two years ago admitted to the emergency room with progressively increasing chest pain . After transtorasic echocardiography, he was referred to our hospital with the diagnosis of aortic dissection. Computerized tomography angiography, coronary angiography, and ascending aortography were performed, and the patient was diagnosed with the pseudoaneurysm of the ascending aorta. The patient was taken into operating room urgently. The pseudoaneursym originated from site of the aortic needle entry. We performed graft interposition with dacron tube graft with number 26 under total circulatory arrest and deep hypothermia. Postoperative recovery was completely uneventful, and the patient was discharged on the postoperative day 7. An effective and adequate surgical approach should be performed due to the risk of rupture and mortality. Keywords : Aorta; coronary artery bypass; aneurysm, false
