Renovascular hypertension and ischemic nephropathy due to renal artery stenosis requires treatment. In patients who were not treated or in whom treatment was unsuccessful, complications that compromise quality of life and even threaten life should be awaited during the natural course of the disease. Nowadays, renal artery imaging and hemodynamic studies provide valuable information for planning of the appropriate treatment and successful endovascular interventions make progress in treatment of the disease. However, in patients who are planned to be operated for aortic disease and in whom endovascular treatment is unfeasible or the procedure was previously unsuccessful surgical treatment has still great importance. We present the surgical procedure and its results in a patient in whom natural course of the disease continued following endovascular intervention and rapidly proceeded to end stage renal failure.
