Isolated iliac aneurysm is a rare condition associated with high mortality than the other
aortic aneurysmal disease. The risk for rupture is high due to asymptomatic progression and late diagnosis.
Repair proposed for 3.5 cm or larger aneursymal diameter in literature review. Although
endovascular repair is the alternative treatment option according to surgical treatment at anatomically
suitable anuersym, patients should be followed up for long term at this procedure. Elective
surgical treatment"s mortality and morbidity is remaining low with carefully planned operative approach
and technical detail. 72 years old male patient presented with pelvic pain. Evaluation of his
digital subtraction angiography (DSA) displayed left common iliac artery aneursym with approximately
18 cm aneursymal diameter. Patient operated in elective condition and Dacron tube graft
interposition was performed. After an uneventful ICU and ward stay he was discharged from the
hospital on the 5th postoperative day in good condition.
