EVOLUATION OF ETHIOLOGICAL CLASSIFICATION, RISK FACTORS AND THE TREATMENT OPTIONS OF THE PSEUDOANEURSYMS OF THE PERIPHERIC ARTERIES: CLINICAL EXPERIENCE OF 104 CASES

1Gaziantep Üniversitesi Tıp fakültesi Kalp ve Damar Cerrahisi Anabilim Dalı, Gaziantep Purpose: The aim of the study is to evoluation of ethiological classification, risk factors and the treatment options of the pseudoaneursyms of the peripheric arteries within litheratur and our expenences.

Methods: We operated 104 patients for pseudoaneurysm ofperipheric arteries between January 2000 and February 2005. Common and superficial femoral arteries are the most often settlement artery (56patients 53.8%). The ethiological classification of our patients are; 29 patients sharp insturment or gun shot injuries (27.9%), 8 patients traffic accident (7.7%), 1 patient landsıide accident (0.9%),6 patients from the recent greft anastomosis (5.8%), 41 patients A-V fistula that were performed for chronic renal failure (39.4%) and 19 patients during diagnostic angiographic evoluation (18.3%). Cutting the pseudoaneurysmatic segment and restoration of the proximal and distal segments by primary end to end anastomosis or safenous vein interpozition is the basic treatmentapproach.

Results: Reason of primer ethiological infection secondary wound infection was developed at 8 cases. At 6 of them the culture was (+) and two of them was gone to debridment for this reason.In 4 patients tissue flebs were used to close the large tissue loss. In all patients the grafts patency were checked with clinic evoluation and Doppler USG. While clinical follow- up, recuren pseudoaneurysm was seen in 2 patients.

Conclusion: Meanwhile if the ethiological reason is iatrogenic or traumatic in pseudoaneurysm early definition and early treatment is very important for the patients health. Even if the treatment time delays, it is getting much harder that treatment and complication risks are increasing, also the tissue and/or loss risk is increasing.
