Kateterizasyon Sonrası Gelişen İyatrojenik Femoral Arter Psödoanevrizma Tedavisi Deneyimlerimiz
1Kalp ve Damar Cerrahisi AD, Başkent Üniversitesi Tıp Fakültesi, Ankara2Radyoloji AD, Başkent Üniversitesi Tıp Fakültesi, Ankara Objective: While interventional treatment methods currently have a great role in the management of femoral artery pseudoaneurysms, conventional surgical methods are still important in the treatment strategy. We aimed to review the current approach for the management of pseudoaneurysms. Material and Methods: The records of 97 patients with the diagnosis of iatrogenic femoral pseudoaneurym between 2002 and 2010 were reviewed. Fifty nine patients were managed with ultrasound guided compression or glue injection and 38 patients underwent surgery. The aneurysm diameter, time of hospitalisation and postoperative complications were recorded. Results: The patients that did not undergo any surgery had a mean hospitalisation time of 0.8±1.3 days while in the surgery group it was 3.2±1.4 days. In the compression and glue injection group 8 patients necessitated surgical intervention in the follow-up (13.5%). The indication for surgery was drainage of hematoma in 3 patients, femoral artery occlusion with glue in 3 patients and treatment failure in 2 patients. In the surgery group 3 patients suffered from hematoma formation and one patient was explored again because of lymphatic drainage. Conclusion: In the recent years a variety of management strategies are being proposed for pseudoaneuryms and spontaneous thrombosis is also reported. However, because pseudoaneurysms are unstable lesions, surgical intervention with low mortality and morbidity is still an important option in the management of femoral pseudoaneurysms.
