THE MIDTERM RESULTS OF THE EXTRA-ANATOMIC FEMORO-FEMORAL BYPASS GRAFTING

An alternative surgical procedure in the management of unilateral aortoiliac occlusion is the extra-anatomic femoro­ femoral (F-F) bypass, especially in high risk old patients and in sexualy active patients in which sexual dysfunction is feared.

in this study, the indications, complications, mortality and patency rates of 19 F-F bypass grafts completed a 5 year period were analyzed. in our clinic elective surgical management was done in 18 patients and 1 emergency F-F bypass procedure was applied after coronary angiography only in one patient. Additionally, femoro-distal bypass grafts were done in 5 patients.

in our series, early graft occlusion (in 30 days postoperat ive period) was seen only in 1 patient (5.2%) and late graft occlusion (in 5 year period) was seen only in 4 patients (21%). Total cumulative patency rate lor F-F bypass grafts was 71.4% at 5 years. The mortality rates were 5.2%, 21% lor 30 days early postoperative period and 5 years res­ pectively. Early postoperative complications were seen in 3 patients; 1 was renal, 2 were inguinal wound infections. Our results alsa show that extra-anatomic F-F bypass procedure in the management of unilateral aorto-iliac occlusion especially in high risk patients is a good alternative management.
