Semptomatik ve Asemptomatik Hastalarda Karotid Arter Aterom Plak İçeriğinin Karşılaştırılması

Abstract
Objective: Homocystein, von Willebrand factor (vWF), fibrin and C-reactive protein
(CRP) values were measured in carotid artery plaques from symptomatic and asymptomatic patients
with carotid stenosis. The values were evaluated between groups. Material and Methods: 36
patients who underwent carotid endarterectomy, aged 54 to 77 (mean age: 64.16, 23 male, 13 female)
were included in the study. 20 of them were symptomatic, 16 asymptomatic. Homocystein,
fibrin, vWF, CRP were measured from samples taken from carotid plaques converted into supernatant.
SPSS for Windows 10.0 programme was used for statistical analysis. P value less than 0.05
was accepted significant. Results: Homocystein value in symptomatic patients was 4.036±1.19
μmol/L, in asymptomatic patients, was 3.647±0.772 μmol/L (p=0.519). Fibrin value in symptomatic
patients was 227.54±38.81 mg/dl, in asymptomatic patients, was 88.26±5.93 mg/dl (p<0.001). vWF
in symptomatic group was 59.33%± 9.13%, in asymptomatics, was 42.83%±5.33% (p<0.001). CRP
in symptomatic group was 0.33±0.13 mg/dl, in asymptomatics, was 0.26± 0.091 mg/dl (p=0.033).
Differences in parameters, except for homocystein, were statistically significant. Conclusion: There
was no relation between homocystein level an symptomatology. Amount of fibrin in the carotid artery
atherosclerotic plaques increases with hemorrhage that may cause critical stenosis and disrupt
plaque and so cause microemboli. Modification of the mentioned factors can change the progress
of the disease.
