Açık Kalp Cerrahisi Sonrasında Derin Ven Trombozuna Bağlı Masif Pulmoner Emboli

Acute massive pulmonary embolism usually results in death and with nonspecific symptoms it is hard to diagnose. The patient who underwent open heart surgery admitted to our hospital with the complaints of dyspnea. Transthoracic echocardiography revealed right atrial mass reaching pulmonary artery. Emergent surgery was planned due to the patient"s progressive hemodynamic instability. During canulation hemodynamic detoriation was followed by cardiac arrest. Under cardiac resuscitation aortic and bicaval canulation was performed and cardiopulmonary bypass was commenced. No mass was found when the right atrium was exposed. Main pulmonary artery was exposed and thrombus was found. Thrombectomy was performed with the aid of forceps and suction. Cardiopulmonary bypass was terminated without any complication. The patient was transferred to the intensive care unit and extubated at postoperative 6th hour. As in our case report surgical embelectomy is an efficient treatment and it should be considered as an life saving method.
