Derin Ven Trombozu Tanısında D-Dimer Testinin Bir Tarama Testi Olarak Etkinliğinin Araştırılması

Abstract

Objective: The study aims; Efficacy of D-dimer as a screening test is revealed in diagnosis of the deep vein thrombosis by using to compare with Doppler ultrasound data. Material and Methods: Quantitative D-dimer analysis results and Doppler ultrasound data of 138 patients with prediagnosis acute deep vein thrombosis were retrospectively evaluated. Patients with complaints more than one week and having chronic deep vein thrombosis were excluded from the study. Doppler ultrasound examination was performed from above knee in all patients. To detect the Plasma D-dimer levels as a quantitative analysis, a polystyrene microparticle agglutination test was used. The prevalence of deep vein thrombosis, sensitivity, specificity, positive and negative predictive values in two patient groups were calculated evaluated by both methods. Results: The prevalence of deep vein thrombosis was found as 39.8%. When Doppler ultrasound data were taken as a gold standard, the results of sensitivity, specificity, positive predictive value and negative predictive value of D-dimer test were respectively found as 89.0%, 57.8%, 58.3%, and 88.8%. Conclusion: The specificity value of D-dimer test in predicting the deep vein thrombosis has a lower value as in the literature. Therefore, it would be wise to say that only D-dimer test may not be effective method while predicting of thromboembolic cases.
